Introduction

The Government of Ontario has a process to allow patients and their caregivers to contribute to the province’s drug review process. Ontario’s Committee to Evaluate Drugs, in their consideration of drug approvals and funding, is interested to understand the real impact that diseases and treatments have on the daily lives of patients and caregivers. The way patients and caregivers contribute to the process is by providing their personal experiences to registered Patient Advocacy Groups. When a drug is up for review in Ontario, registered Patient Advocacy Groups can make a submission on behalf of a group of patients. The goal of such submissions is to help the Committee understand the personal implications of the illness and its treatments. The Lung Health Foundation is pleased to be registered as a Patient Advocacy Group to help provide patients and their caregivers a voice in the consideration of medications related to lung disease.

The following survey has been created to collect information from individuals living with a chronic lung condition and from their caregivers. The questions have been designed to determine how this lung condition affects your lives. Information received will be submitted to the Canadian Agency for Drugs and Technologies in Health to inform their consideration of drugs that are up for review which are indicated to treat lung conditions. This is an important way for your voice to be heard with the Government of Ontario to help with key decisions that affect lung health. To add your voice to this process, please complete this survey. Further, to help improve our reach and the patient submission process, please share the link to this survey with anyone you know who is living with or caring for someone with a chronic lung disease/condition. The information you voluntarily provide in this questionnaire will be used to better inform our submissions to the Canadian Agency for Drugs and Technologies in Health. Your responses will be combined with those of many others and summarized in a report to further protect your anonymity. No personally identifiable information will be shared within the submissions.
